Youth and Mini-Game Player: Membership Application

2019/20 Season; (1st September 2019 – 31st August 2020)

The purpose of this form is to make the “Player” a Member of Harwell & Hendred Youth FC for the season 2019/20. The vast majority of Members, if not all, will also have (existing players) to complete new League Player I.D. forms for the Oxford Mail Youth Football League or Oxford Mail Girls Football League, as the case may be.

Firstly, please tell us a little bit about the applicant...

Pictures of teams, squads, players and Managers/ Coaches with players may be taken to use on the Harwell and Hendred Youth FC website or in Club publicity material. 

  1. All parents should take the short FA Respect Guide for Parents course at http://www.thefa.com/respectguide/ so they are aware of the standards of behaviour expected by everyone across the football community. Remember that children play and develop better in a friendly, encouraging atmosphere where they play for fun. [See also Parents Code of Conduct at the www.hhyfc.info website.]
  2. The above information will be used solely by Harwell and Hendred Youth FC for the purpose of membership and club matters. The information will not be passed on to any third party and will be held in accordance with the Data Protection Act and the HHYFC Data Protection Policy and Privacy Notice - available at Club website here ( http://www.hhyfc.info/index.php/home/club-documents )
  3. Declaration In the event that my son/daughter is injured while playing football or at football events and I am not present and cannot be contacted on the above number at the appropriate time, then I hereby give my consent for my child to receive appropriate medical attention and, if relevant, emergency services to become involved.
    I, the parent/guardian, consent to the named player being registered with Harwell and Hendred Youth FC as a player. We (the player and I) will abide by the Constitution, Rules and Codes of Conduct as laid down by Harwell and Hendred Youth FC and understand that failure to do so could result in registration being withdrawn or disciplinary action taken.
